"I needed blank forms of Power of Attorney and Medical POA. They promised to send me the forms by mail and also by email. I called almost daily bc I never even got the emails to 2 different addresses. Finally after 2 weeks of bugging them daily I got the forms via email. The blank form via mail never arrived. They would not let me go pick up bc they are short staffed and don’t allow walk-ins"
