"Never was able to make it through the door. Unfortunately there was a miscommunication that ended in my appointment being cancelled. I was also at fault for assuming the person who was contacting me about scheduling and paperwork would be the one to send said paperwork. However, I do feel that is a natural assumption. So that is unfortunate. Anywho, be aware of that in case you cant find your paperwork on time!"
