"Several years ago I started looking at going to a Chiropractor and visited this business several times. At one point I was needing to set up an appointment and couldn't get anyone one the phone to set one up , so I went to the website to make the appointment and when it came time for my appointment no one was at the office. No phone calls to reschedule or anything. So I went to another Dr. in another city that was recommended to me. Recently I wanted to give this business a second chance and called the office for my appointment and was instructed to fill out the new patient forms online. I submitted everything that was required and the day before my appointment I called and spoke with the receptionist to verify my appointment and that the forms that were submitted via this business' website had been received. I was told everything was in order and she would see me at my scheduled appointment time. Less than an hour before my appointment I received a call that the business hadn't received the paperwork and that unless I resubmitted the paperwork that I could not be seen at my appointment time. I resubmit the forms and show up to my appointment and gave my insurance card to the receptionist was asked for my Social Security Number, I politely asked why my SSN was needed and was told so that they could look up my Insurance info. I told her that she had a copy of my insurance card and I would provide a copy of my drivers license but would not provide my SSN. I was told that I could pay cash or provide my SSN. I left without being seen. I called my insurance provider and was told to never give your SSN to a provider that the insurance can be verified by insurance member ID. I hope that they are successful in their business but do not recommend them."
