"This was a horrible experience. Do not go here. Go to the Corvallis Clinic. It’s like they want people to fail their physical and you do not get a refund on cost if you fail. If you have to go to them, just lie about everything because if not then they won’t pass you and you won’t have a job and you won’t get reimbursed for the cost. The only nice person was the RN that worked under the nurse practitioner. Even the front desk workers were rude as hell. They will also lose your email if you have to email them the physician form. You have to keep calling them till they receive it so you can finally make an appointment with them. Just go to Corvallis."
