San Francisco Community Clinic Consortium (SFCCC) is a health institution located at 170 Capp Street in San Francisco, California, United States. SFCCC is dedicated to developing innovative programs and advocating for policies that increase access to quality community-based primary health care. Serving over 112,000 low-income, uninsured, and medically underserved individuals annually, SFCCC's 12 member clinics provide comprehensive primary, preventive, and ambulatory care services to diverse populations including low-income, homeless, immigrant, LGBTQ, HIV-positive, and senior San Franciscans. The clinics offer services in up to 20 languages and operate on a sliding scale based on income, ensuring that no one is turned away due to inability to pay. Additionally, SFCCC's Street Outreach Services (SOS) provides mobile health services to homeless individuals, while Veterinary SOS offers free veterinary care to the companion animals of homeless San Franciscans. SFCCC has also been selected as a National Health Corp (NHC) operating site, accepting applications for emerging health leaders interested in serving underserved communities in San Francisco.
