"If you have the need for a service dog or an emotional support animal for a diagnosed health condition with an extensive history, they will not write you a letter or complete disability paperwork. They also will not show you the policy against it or write a letter stating that they cannot complete the paperwork. I was given an excuse about not being licensed to complete the forms or the letter. It makes me question their licensure to be able to provide services at all. Please save yourself the trouble and find another clinic or medical provider."
