DBA Bluegrass Community Health Center, located at 224 North Martin Luther King Boulevard in Lexington, Kentucky, is a patient-centered medical home that has been serving Kentucky families since 2001. The institution provides comprehensive primary evidence-based care for adults and children. The mission of Bluegrass Community Health Center is to provide an accessible Health Home that promotes better health, better life, and serves all people with care and respect. Medications can also be delivered to Fayette County homes if needed. The operating hours are Monday to Thursday from 8am to 8pm, Friday from 8am to 6pm, and Saturday from 8am to 4pm.
