"I'm not happy with this Doctor's office at all. I needed a form filled out to waive student loans because I'm disabled and I'm 68 years old. They would not fill out the medical form. Stated their office doesn't do that. I'm like you are my primary care physician and this form is to be filled out by my doctor. I'm not going back to them again. That was just uncalled for."
