"5 months ago I gave this place a 5 star. My mother my wife and I could not have been any happier. Over the last 3 months it went from 5 stars to 2 stars and that's being generous. There is no accountable at all when something is done wrong. There is a handful of great workers there. But when there is a problem all we get is we are training new staff. One would think the priority would be to hire more experience staff. I wash my mother's clothes two to three times a week and it's amazing how overnight 8 outfits, shoes and socks can disappear. They dress my mother's room mate with my mother's clothes. It's just been real disappointing the changes that has accured. When trying to address issues they get swept under the rug. Hardly any staff wear masks going from room to room. I'm not going to even mention the mismanagement of medications given to her that caused her to be bed ridden for the last 60 days because she was addicted to them. However, they were told from day one not to give those to her because she was addictive to them and had been hospitalized over them. . Sad part is not even an apology for it."

"I highly recommend Self Health Care and Rehab Center! My father spent a little over a year living there. The staff loved and cared on him so well. I felt they truly cared what impact (mental & physical) the COVID pandemic was having on the lifestyles of the residents, as well as the concerned families not able to visit with their loved ones. They would have activities to help keep the residents in good spirits throughout the pandemic. They truly care about their residents."
