"My friend arrived at this facility Tuesday, August 8th. She has deteriorated since she has been there. No shower, impacted now, and in pain. The nurses documentation is false! Your loved one deserves much better than this place. The joining bathroom has had urine on the floor and wall for 2 days. All of this was reported to supervisor to no avail."
"After reading a few good reviews, I thought it can't be bad. Smelled clean and was bright. That's the end of anything positive. Unfortunately, my dad was there for 3 weeks before I could get him moved. He came for rehab after a 2 week hospital stay to help build his strength up. He left in worse shape. He had 1 shower the beginning of his stay and not another one until a week and a half in. He smelled awful. My son and I had to keep helping him change clothes because he was filthy every time we visited. He said he would call for help if he had an accident in his bed and at least on one occasion they made him lay there for an hour in his own urine. PT was maybe 1 hour a day then he was sent to his room and confined to his bed or wheelchair. He said they got mad if he tried to get up. They are understaffed and difficult to get answers from. Always shooed me away and said they would come to the room. Tried to call the administrator but was told she was in a meeting. I left a message to return my call, she never did. The furniture in the rooms are antiquated and highly uncomfortable. My dad cut himself multiple times on the bed. Found him one day with dried blood all down his leg and on his sheets (stayed for 2 days). Wounds were barely cared for and bandages left on for days. Patient bathrooms were old and nasty. My dad said many of the staff members were always arguing. I witnessed this myself, I even heard a male staff member yelling at a patient in the cafeteria, then later complaining to a nurse how it wasn't his job, etc. She yelled back at him. All outside my dad's room. They are noisy and have no consideration for quiet time in the evenings when people try to sleep. My dad was miserable and kept begging me to get him out. When I came to pick him up, it smelled heavy of bleach and many of the staff were wearing masks. Walked in and my dad had "a head cold". I asked multiple staff if COVID was going around. They completely ignored my question or referred me to their web page. Nothing was posted on the doors or website as I could find. Brought my dad to his new assisted living where he tested positive for COVID two days later, which he passed along to myself and my son. I complained to the social worker a week before he was discharged and she claimed she was filing a grievance report. Never got a follow-up if that even occurred. Had one meeting two weeks in to even get an update on his care, which was minimal. One evening they forgot to bring his dinner. I had to hunt it down for him. Thankfully I was there that evening. I could go on and on. Please do NOT trust this place to care for your loved one."
"Fort Walton Beach Rehabilitation center , staff and administrator Ashely Hodge in particular took great care of my family member while she was there being out of area I had nothing to worry she was in great hands, I wish they we're closer to me I'd had my mom there instead where she was at locally. "Our friend and neighbor was there for rehab after a stroke. The staff was very caring and attentive. They also allowed his friends to visit often and in large numbers to bring a little bit of home to him while he was recovering. Some have rated FWRC low because of the response time to the call button, but these people have unrealistic expectations. They do very well with the staff they have. They really care for their patients and it shows."
"Sent from Yahoo Mail on Android My mother was sent to Ft Walton Rehab Center by Emerald Coast Hospice and her caseworker. She was only there for a Respite stay of 5 days starting on 1/20/2023. The first night she was there, she fell and the staff there called 911 instead of calling Emerald Coast Hospice first as well as her caseworker. My mother also wore the same pants and shirt for 2 days, I had put clean clothes in the top and middle drawers in the nightstand next to bed 1/22/2023. On 1/23/2023 the doctor from the facility came in to examine my mother and when I commented that Mom was there for Respite only and she was to stay there for 5 days only and he replied "Ohhhhh", like he didn't know she was there only for Respite. I got the impression that he thought she was going to be a permanent patient. After all said and done, I requested my mom be discharged on 1/24/2023 instead of 1/25/2023 with the assistance of her caseworker at Emerald Coast Hospice. I got her back home on 1/24/23 and Mom had to use the restroom and when I helped her to pull her Depends off I noticed she had a really bad diaper rash, which she didn't have before I took her to Ft Walton Rehab Center on 1/20/2023 !! I'm not happy with the way my mother was treated at this facility Ft Walton Rehab Center. Unsatisfied son. Sent from Yahoo Mail on Android"
