Please note that we do not accept insurance for exams, and all fees are self-pay. Vaccinations required for the exam can be administered at the Health Department or your pharmacy. Refunds will not be issued for disqualification or incomplete forms due to abnormal results.

For vaccine requirements, we offer Tdap and Influenza vaccines in our office, while others must be obtained elsewhere. It is essential to have at least the first dose of any required vaccine completed before submitting Form i-693.
