"I’ve had the worst experiences from this practice. The most recent is simply asking for a wellness check form to be completed which is a requirement from my employer. I had already visited for an annual physical. This form was as simple as completing 10 or less fields with my lab information. They said I MUST come in for a form completion which they would charge a normal office visit for. They try to milk the insurance company for every single penny. It’s a complete joke."
