"This office needs to become familiar with a DD2813 form and the requirements if they plan to conduct business in a highly military populated community. I have attempted to have this form filled out at two visits. Even after trying to explain that the form could be completed, but a class level would need to be annoted, the Dr. still would not complete the form. I have one POSSIBLE filling, which would place me as a class 2, and still deployIable. Yet, I've had to waste two personal leave days, due to this establishments inability to educate themselves on these requirements."
