"I went to the counseling agency to have an evaluation completed for an upcoming procedure I am having done. The paperwork needed to be send to the doctor before moving forward. It has been months and the doctor's office has not received the paperwork. I have made multiple attempts to have this completed with no success. The counselor was professional during the session, but the lack of response on completing the paperwork is inexcusable. Will never return nor refer them business. Disappointing."
