Element Care

Element Care, located at 62 Market Street in Lynn, Massachusetts, is a health institution that offers PACE (Program of All-inclusive Care for the Elderly) services to help seniors live independently in their homes for as long as possible. Our expert staff provides personalized care to promote wellness, engagement, and purposeful living through a variety of activities and social opportunities. Participants in our PACE program report living longer, healthier lives with fewer hospitalizations.

We are expanding our services to include innovative technology and telehealth options to enhance continuity of care for older adults. If you are 55 or older, you may be eligible for PACE, with medical costs covered for those on MassHealth and Medicare. Our caring team of professionals, including doctors, nurses, therapists, and social workers, work together to provide comprehensive healthcare for adults who require complex, coordinated care.

Enrollment in PACE at Element Care can be done online for the safety of our patients and staff. We offer a second family-like environment where all healthcare services are coordinated for the benefit of our participants. To report a compliance concern, a confidential Compliance Tip Line is available. Personalized care, adult day health centers, eligibility information, and enrollment processes are all part of the services offered at Element Care.
