"She prescribed an abundance of medications including SSRIs that had warning labels that indicated I was high risk for suicidal ideation. She never went over the risks and/or possible side effects with me. When this started actually happening she never informed me that these medications carried these warnings. I nearly died. It is documented. She has been reported."
