About Sub Acute Care Unit Townsville Hospital
Welcome to the Sub Acute Care Unit at Townsville Hospital in Queensland, Australia. Our establishment is a leading health institution that offers restorative and rehabilitation services to the community following medical or surgical issues. Our goal is to help patients regain their independence in daily living through specially designed programs and activities within our unit.
As a long-stay unit, we understand the importance of providing a comfortable environment for our patients and their families. Our facility includes community areas where patients can socialize and spend time with their loved ones. We encourage patients to dress in the daytime and wear appropriate footwear for their therapy programs.
For older patients, functional disability is often more critical than their medical problems. To address this, we offer a Comprehensive Geriatrics Assessment (CGA) with a multidisciplinary approach. Our team assesses and manages patients each weekday, with regular medical ward rounds and family meetings as part of the discharge planning process. We work closely with the Transition Care Program to ensure a successful discharge and ongoing functional improvement in the community.
In addition to our restorative services, we also provide inpatient rehabilitation at our custom-built 43-bed facility. Our aim is to promote independence in activities of daily living and help patients regain functional capacity after illness or injury.
For those who require outpatient rehabilitation, we offer specialist assessment and management through our various clinics. Our clinicians work closely with patients to create goal-directed rehabilitation programs that maximize functional independence.
We are proud to collaborate with the oncology team to offer the OncoGeriatric assessment clinic. This clinic focuses on early identification and management of frailty and geriatric syndromes in older adults with newly diagnosed cancer. Our specialist geriatricians perform comprehensive geriatric assessments and provide individualized recommendations.
To ensure accessibility for rural patients, we offer Telehealth services. This allows us to connect with rural facilities and provide services such as frailty and dementia diagnosis, assistance with capacity and placement decisions, and assistance with the formal diagnosis of dementia syndromes. Our Telehealth consultations can be performed in the patient's home, saving travel for frail patients.
To make a referral, please address it to the Rehabilitation Consultant through a letter format or HealthPathways. Email is our preferred delivery method, and we will strive to schedule an appointment or review the patient at the earliest convenience.
