Welcome to Budyari (Miller-The Hub) Community Health Centre, located in the suburb of Miller in south-west Sydney, Australia. As one of the most socio-economically disadvantaged suburbs in the greater Sydney area, our institution is dedicated to providing comprehensive health services to the local community.
At Budyari, we follow the principles of the Patient Centred Medical Home model, ensuring that our services are tailored to meet the specific needs of our Aboriginal patients. We are proud to house the South Western Sydney Local Health District's Aboriginal Chronic Care Team, who work tirelessly to coordinate care for our patients with chronic health conditions.
In addition to our chronic care services, we offer a wide range of visiting health services delivered by specialists and allied health professionals. Our specialist clinics include endocrinology, nephrology, cardiology, gastroenterology, and addiction medicine. We also provide on-site services such as drug and alcohol support, mental health services, dietetics, exercise physiology, and a general practitioner who attends once a week. Social services such as Centrelink and Legal Aid also regularly visit the centre to provide assistance to our patients. We are excited to soon have a full-time social worker to support our Aboriginal Chronic Care Program.
We have established strong connections with Gandangara Health Service, which provides a GP who visits our centre once a week. We have reciprocal agreements with them for referrals and joint care coordination of shared clients. These partnerships have been developed over many years of negotiation and trust-building, ensuring that our patients receive the best possible care.
Our chronic care program is managed by a dedicated team that includes Aboriginal health workers, a clinical nurse specialist, a renal clinical nurse consultant, a dietician, and a service manager. We currently have approximately 200 patients enrolled in our program, receiving active care coordination and case management. We conduct health assessments, develop collaborative care plans with GPs, communicate directly with other care providers, and connect patients with social services when needed. Our focus on improving referral pathways from hospitals ensures that patients receive timely engagement post-discharge.
At Budyari, we understand that addressing the social determinants of health is crucial to improving overall well-being. We assess the full range of our patients' needs, including housing, which is a major concern for many in our community. By expanding our scope to include the social determinants of health, we engage in preventative care and provide a person-centred approach to healthcare.
