Located at Georgia 119, Riceboro, Georgia 31323, the Liberty County Health Department is part of the Coastal Health District of Georgia, serving the communities of Bryan, Camden, Chatham, Effingham, Glynn, Liberty, Long, and McIntosh counties. Our mission is to enhance the health and well-being of our residents by preventing disease, promoting health, and preparing for and responding to disasters.

Hurricane Registry for Functional, Access, or Medical Needs:

Hurricane season runs from June 1 through November 30. Our Hurricane Registry is open year-round for residents who may need transportation or medical assistance during an evacuation. If you meet the criteria, you can apply by calling toll-free at 1-833-CHD-REGISTER (1-833-243-7344). Please note, applications close 72 hours before the arrival of tropical storm-force winds.
