As a proud Federally Qualified Health Center (FQHC), Family Health Network (FHN) has been dedicated to serving Cortland and neighboring counties since 1972. We are committed to being the primary source of healthcare in three of the five communities we serve, offering critical sliding scale adjustments to ensure accessibility for low-income individuals and families throughout the region.
At FHN, we continuously strive for excellence in healthcare delivery. Our recent implementation of a state-of-the-art Electronic Health Record (EHR) system and a user-friendly Patient Portal exemplifies our dedication to enhancing patient experience and care coordination. Our quality of care has been recognized by the American Heart Association, receiving both the Gold and Silver Awards for the Target BP initiative, and we are proud recipients of Community Health Quality Recognition (CHQR) badges.
Our innovative Model of Care, developed in partnership with Coleman Associates, focuses on team-based care. This transformation has led to significant performance improvements in two of our five health centers. Additionally, our Mobile Medical Unit, accessible by wheelchair and equipped with a rear exam room lift, extends our reach to those in need.
FHN is committed to inclusivity, accepting all major health insurances, including Excellus, to ensure comprehensive coverage for our patients. As part of our dedication to community health, we actively participate in initiatives like the Cortland County Health Access Task Force, working to provide vital COVID-19 information across New York.
