"They lost my paperwork we had just filled out 1 month ago. Showed up for an 8 am appointment to fill it out all over again this is total incompetent. You can't put my paperwork into the computer and have it for every visit I also have a doctors appointment on Monday hopefully I don't have to fill out the paperwork all over again or else I'll be leaving again"
