"This medical office is a joke! They forgot to stamped the physical form. Staff happened but you need to fix it. So I got their fax number and send it to be stamped. Nothing happened. After weekend I called and they gave me another fax number. Nothing. I called next day and explained the situation and gave me the third fax number. If you give a fax number, you are responsible to operate it, right? Such lazy imbeciles!!!"
