SmartCare is a cutting-edge mental and behavioral health program located at 1012 Main Street in Ramona, California. As a part of Vista Hill, we work closely with primary care teams in rural health clinics in north inland and south east communities of San Diego to provide integrated care for our clients.
Research has consistently shown the benefits of integrated health care models for both patients and providers. In response to the high risk of negative behavioral health outcomes in rural San Diego communities, SmartCare was established to address these challenges with a collaborative care approach.
Our team utilizes the Collaborative Care Model (CoCM) developed at the University of Washington to treat common mental health conditions such as depression and anxiety. By offering evidence-based practices in a client-centered, low-barrier setting, we aim to improve access to care, reduce stigma, and enhance overall health outcomes for rural residents.
