Our team of dedicated professionals provides comprehensive assistance to individuals of all ages in need of behavioral health support. Our services are designed to improve the functional abilities of individuals diagnosed with various physical and mental health conditions. We accept Medicaid, Managed Medicaid, Medicare, and most private insurance companies, as well as offer a sliding fee scale based on eligibility.

Our Upstate Cerebral Palsy Care Management Program offers personalized clinical and social support aimed at improving the health and well-being of the community. We provide access and care coordination to health and community support services to enable individuals to live healthier, more productive lives. Our Behavioral Health Navigators are available to connect individuals with services and resources to improve overall health and well-being, including housing, food, financial assistance, and more.

Our residential and housing supports at Upstate Caring Partners promote the dignity and worth of every individual. Our programs are designed to assist individuals in improving functional living skills and promoting movement toward living independently in a less restrictive community setting. We offer 24-hour support services in our community residences, as well as short-term respite living opportunities for those in need.

The Image Center provides a safe place for individuals to socialize and participate in various activities. Children are referred to Upstate Caring Partners for mentoring services, and our family peer advocate program offers support to families with children facing medical, mental, or behavioral challenges. Peer Support Workers, who have lived experience and have successfully recovered, are available to help individuals in their recovery process.

For service members, veterans, and their families, we provide specialized support to address their unique needs. Our goal is to empower individuals to live the healthiest, most fulfilling lives possible, and we are dedicated to providing high-quality care and support to all those we serve.

"My adult son has had three appointments canceled with these people. He is depressed and suicidal and can't get in to see a doctor so he can get additional medication. How can they justify doing this to a person?? Mental health is not a priority in this country and it's places like this who are responsible for the poor state of our mental health services."
