"Schedule a vaccine and got there and no1 had pulled the vaccine out. Needs 1.5hrs to unthaw, got there 20min before my appt like they requested and had to come back hour after original appointment. Then the paper work asked for SSN, and DL# along with mothers maiden name…….I’ve worked in medicine for over a decade and NEVER needed anyones mothers maiden name. Unprofessional and really not where I will be going again."
